All expecting mother’s that are member of Social Security System (SSS) may take advantage of their maternity benefits by first informing the SSS for their upcoming delivery.
The process varies whether if you are employed or self-employed. If employed, you can simply notify your employer immediately as soon as you confirmed that you are pregnant by accomplishing the SSS maternity notification form and the proof of your pregnancy. It will be submitted by your employer to notify the SSS along with the form.
However, for those unemployed, self-employed and voluntary members, you have to notify the SSS directly by visiting their nearest office in your place and by submitting the maternity notification form that you can also get on the SSS office or online.

After informing the office the SSS personnel will give you the following form to fill up:
1.SSS Maternity Notification Form- This must have received date stamp on it with the signature of the SSS personnel.
2. SSS Maternity Reimbursement Form- Make sure that you fill out this form completely and submit it including the other forms with the requirements after your delivery.
3.Complete Obstetrical History Form- This is to be filled up by your attending OB.
Several months after the delivery, the member can return all the given form to SSS office along with the requirements that includes the expense records from the Hospital where the member gave birth. The hospital expenses can be asked to your attending OB to request.
After all the requirements are submitted, verified and received by SSS, the member need to wait for 1 to 2 months for the check to arrive.
